Member Eligibility

  • Ages 16 and over​

  • Physical and mental condition

  • Approval of Primary Care Physician (PCP)

  • Approval from insurance like MassHealth

  • Requires assistance with at least one daily activity like

    • Bathing

    • Dressing

    • Toileting

    • Eating

    • Walking​​​

Caregiver Eligibility

  • Can be family or non-family member

  • Must live in the same house as member

  • Must provide the required physical and nursing assistance

  • Must not be spouse of member

  • Must not be legal guardian of member
